Below you will find the most common forms used in the Recreational Sports Department. You may drop the filled form at the Student Recreation Center front desk during our normal hours of operation or you may send it as an attachment to recsports@utep.edu Note: All forms must be signed and sent before the entry deadline date has passed, forms received after that will not be admitted unless you have the prior approval of the Recreational Sports Department. Membership forms MUST be turned in to the membership office room 102B and cannot be emailed of faxed.
