We are excited to welcome your students and guests to our campus. To help ensure a safe and enjoyable visit for everyone, all participants are required to complete the appropriate University of Texas at El Paso (UTEP) forms prior to their visit.

Please download, complete, and submit the following documents for all students and adult chaperones attending the visit:

• Release and Indemnification Agreement (Minor)
• Release and Indemnification Agreement (Adults)
• General Photo Release Form

Completed forms must be received before your scheduled visit date. Participants who have not submitted the required forms may not be permitted to participate in campus visit activities.
