Youth Program Notification Form
Using the link below, please complete a separate form for EACH youth program you want to host.
Forms must be submitted at least three (3) months prior to the start of the program. Form submissions received less than three (3) months prior to the start of the program must include departmental leadership authorization. (UTEP HOPS 5.3.1 Youth Program Notification Requirement)
Note: In the case of youth programs, where a registration fee is administered, Professional and Public Programs will provide administration support and oversee the additional services for a coordination fee when the youth program does not have the means to collect payment through an UTEP approved-system.
