Release Form
Informed consent serves as an agreement in which the interview process is thoroughly explained to the interviewee. By signing the agreement, the interviewee grants The University of Texas at El Paso permission to utilize oral/video recordings for historical and scholarly purposes. The agreement also outlines the purpose of the interview and the rights of the interviewee.

Summary Form
The Institute of Oral History Summary Form provides detailed information about the contents of the interview. The form helps identify the name of the interviewee and interviewer; date, length, and location of the interview; and a summary of main topics discussed during the interview.
