• Application for Graduate Degree:  Application for M.A. graduation / Application for Ph.D. graduation

• Application for Graduate Certificate:  PDF

• Degree Plan Form:  Online Degree Evaluation To generate your electronic degree evaluation, click the link to go to Goldmine, then proceed to the link called “Degree Evaluation” for the current term. You might have to select your program (e.g., Ph.D. in Psychology (General)). Print out the output as a PDF and send it to your Graduate Program Director (Dr. Serafine, kmserafine2@utep.edu) for evaluation.
