Report No Activity

If you did not participate in any outside activity during the previous calendar year, you must affirmatively indicate that fact by completing the No Activity Form. View the instructions for reporting no activity. Click the No Activity Form and please ensure you select the “prior year”.

Generate Disclosure From Request

If you requested approval during the previous calendar year, you can generate a disclosure from your final-approved request. View the Instructions on how to generate a disclosure. Once all sections have been reviewed and updated, please submit the disclosure form.

If you participated in an outside activity but did not submit a request for approval during the previous calendar year, click on Activity Form to begin a disclosure form. view instructions on how to begin the disclosure form. Once all sections are complete, please submit the disclosure form.
