First-Time Adult Passport

Adults applying for their first U.S. passport (age 18 and older) must appear in person at the UTEP Passport Acceptance Facility. Review the checklist below to ensure you bring everything needed for your appointment.

What to Bring

  • DS-11 Form — completed online or by hand  with black ink (do not sign until instructed at your appointment)
  • Original Proof of Citizenship — U.S. birth certificate or Certificate of Naturalization
  • Valid Photo ID — driver’s license, state ID, U.S. passport
  • Two (2) passport photos — 2×2 inches
  • Two separate payment for each fee (details below)

Passport Fees

You will need to make two separate payments — one to the U.S. Department of State and one to UTEP.

  • Department of State (Application Fee):
    • Passport Book: $130 — Expedite: $190
    • Passport Card: $30 — Expedite: $90
    • Passport Book & Card Combo: $160 — Expedite: $220
  • UTEP (Execution Fee): $35 — separate payment required

Accepted payment methods: Check or money order only. Cash, credit, and debit cards are not accepted.

Make sure all documents are original and complete before arriving for your appointment. Do not sign the DS-11 form until instructed by a passport agent.

Passport for 16–17-Year-Olds

Applicants aged 16 or 17 can apply for their first U.S. passport at the UTEP Passport Acceptance Facility. At least one parent or guardian should be present to verify the applicant’s identity and provide consent. Review the checklist below to make sure you bring everything needed for your appointment.

What to Bring

  • DS-11 Form — completed online or by hand with black ink (do not sign until instructed at your appointment)
  • Original Proof of Citizenship — U.S. birth certificate or Certificate of Naturalization
  • Valid Photo ID for Applicant — driver’s license, state ID, or U.S. passport
  • Valid Photo ID for Parent/Guardian — one of the following:
    • Driver’s license
    • Previously issued passport
    • Certificate of Naturalization
    • Current government-issued ID (municipal, state, or federal)
    • Current military ID (applicant or parent/guardian)
    • Matricula Consular issued by the Mexican government
  • Two (2) passport photos — 2×2 inches
  • Two separate payments for each fee (details below)

Passport for Children Under 16 Years

All children under 16 must apply in person at the UTEP Passport Acceptance Facility. Both parents or legal guardians must appear with the child to give consent for the passport to be issued. Review the checklist below to make sure you bring everything needed for the appointment.

What to Bring

  • DS-11 Form — completed online or by hand  with black ink (do not sign until instructed at your appointment)
  • Original Proof of Citizenship — U.S. birth certificate or Certificate of Naturalization
  • Documents Proving Paternity — if applicable
  • Identification Document for Child
  • Parental Consent — Both parents or legal guardians must appear
  • Parent/Guardian ID — one of the following to establish identity:
    • Valid U.S. driver’s license
    • Previously issued passport
    • Certificate of Naturalization
    • Current government-issued ID (municipal, state, or federal)
    • Current military ID (military or dependents)
    • Matricula Consular issued by the Mexican government
  • Two (2) passport photos — 2×2 inches
  • Two separate payments for each fee (details below)
  • If a parent/guardian cannot be present: Include a notarized DS-3053 Statement of Consent form signed by the absent parent/guardian

Passport Fees

    • Passport Card: $15 — Expedite: $75
    • Passport Book & Card Combo: $115 — Expedite: $175

U.S. Passport Renewal

Although you may renew your passport online via travel.state.gov , you may also schedule an appointment with us at the UTEP Passport Acceptance Facility.

Can I Renew? Am I Eligible?

You can use the renewal form (DS-82) if you answer “YES” to all of the following:

  • I can submit my most recent U.S. passport book and/or card with this application.
  • I was at least 16 years old when my most recent passport was issued.
  • My most recent passport was issued less than 15 years ago.
  • My passport has not been mutilated, damaged, or reported lost or stolen.
  • My passport was not limited to less than the normal validity period due to damage, theft, or non-compliance.
  • My name has not changed since my most recent passport, OR I can submit proper certified documentation for a name change (marriage certificate or court order).

If you answer “NO” to any of these, you cannot use the DS-82 form and must apply using form DS-11 or DS-5504 depending on your circumstances.

What to Bring

  • DS-82 Renewal Form — completed online or by hand with black ink
  • Your Most Recently Issued U.S. Passport — must have been issued at age 16 or older and within the last 15 years
  • One recent color passport photo — 2×2 inches, plain white or off-white background, no eyeglasses or uniforms (see travel.state.gov/photos)
  • Proof of Name Change [IF APPLICABLE] — if your name has changed since your last passport (certified marriage certificate, court order, or divorce decree)
  • Two separate payments for each fee (details below)
