How to Request Your Clinical Records

Students who wish to access their clinical record may speak with their current CAPS counselor or make a request at the front desk. A CAPS team member will provide guidance, and if needed, an appointment with a CAPS Director or Assistant Director may be scheduled. You may also ask the front desk for more details.
