Mail Services Forms
Postage Charge Authorization Form
Allows UTEP Departments to track the use of mail charged to department budget cost center. Complete this form prior to mail processing.
Department please make a copy after the form is fill out, in order to keep it for their reconciliation purposes
Mail Services Request Form
Use this form to request: Department Name Change, Request for Mail to Stop, Department Merge or Department Name Change etc. Complete this form and email it to the Assistant Manager at Mail Services Jesus Martinez.
Mail Services Labor Request Form
Use this form if a department needs assistance to process the non-profit permit. Our office will be charging a labor fee. In the event that the mail out will be processed in our office the fee will be remove and the only charge to the department will be the postage.
